Prevalence and Phenotypic Detection of Carbapenem-Resistant Enterobacter Species from the Clinical Specimens of a Tertiary Care Hospital in Bangladesh

Background: Carbapenem-resistant Enterobacter (CRE) is an emerging threat spread rapidly around the world in past few years, posing substantial hazards to human health. Objective: The study was conducted evaluate prevalence of carbapenem resistance and detect carbapenemase producers phenotypically from patients a major tertiary care hospital, Bangladesh. Methods: Prospective cross-sectional July 2018 June 2019, at Dhaka Medical College hospital. Patients admitted different wards, intensive units, visited outpatient department, samples received microbiology department were included this study. A total 350 clinical collected, inoculated, incubated accordance standard protocol. Identification done using biochemical method. Antimicrobial susceptibility testing commonly used antibiotics including imipenem Kirby Bauer disk diffusion All isolates screened for as per CLSI guidelines. Such strains then subjected phenotypic confirmation production by Modified Hodge test. that gave positive screening test further evaluated Metallo-β lactamase (MBL) production. MBL detected Combined Disc Test (CD) combination Imipenem Imipenem-EDTA double disc synergy (DDS) Result: specimens analyzed, which 224 (65.14%) yielded growth. Among them, 28 (12.28%) isolated 12 (42.86%) found be resistant (14.25%) labeled 'Carbapenem-resistant Enterobacter" or CRE. Minimum inhibitory concentration (MIC) determined among carbapenem-resistant isolates. (MHT) performed on showed 9 (75%) enzyme producers. Nine out twelve are Metallo-beta-lactamase combined (CDT) eight (66.67%) DDS Conclusion: finding revealed high species hospital settings well community levels.
